Billing for visits, tests, or supplies that were never provided
Billing for a more expensive service than what was done (upcoding)
Services or prescriptions that were not medically necessary
Someone using another person's insurance card or identity
Payments or gifts in exchange for patient referrals
Falsified, altered, or missing medical or billing records
Misuse of patient information or other privacy concerns
